Clinical Pharmacist, and her supporting technician, have expert knowledge of medications. They have responsibility for the provision of medicines and ensure the safe prescribing and administration of medicines within the In-Patient Unit, including assessment of prescriptions on admission.  

Within the In-Patient Unit, they provide information and solutions regarding managing medicines, including providing medication charts detailing what each medicine is for and how it should be taken. On discharge, they liaise with GPs and community pharmacies to ensure continuity of care with your prescriptions. They are also part of the Living Well Hub supporting patients with information on community pharmacies, tools and services available.
